DDYF – The Diamond House offers community-based residential support options designed to meet the diverse needs of individuals with mental health and intellectual/developmental disabilities. Services are delivered in accordance with North Carolina regulations, person-centered planning requirements, and authorized service plans. Our residential programs emphasize safety, stability, skill development, and community integration while respecting individual choice and dignity.
Alternate Family Living (AFL) – Residential Support
Who It Serves
Individuals with intellectual and developmental disabilities who benefit from a family-based living arrangement.
Service Overview
Alternate Family Living (AFL) provides residential support in a private home where the individual lives with a trained caregiver. Services support daily living needs, community participation, and personal goals in accordance with Medicaid waiver requirements and person-centered service plans.

Admission Criteria
Admission to DDYF – The Diamond House residential services is based on eligibility, appropriateness of placement, and authorization in accordance with NC DHHS and applicable Medicaid or funding requirements.
General Admission Criteria (All Residential Services)
Individuals must:

Have an identified need for residential support services

Have services authorized by the appropriate LME/MCO, Medicaid waiver, or funding source

Have a current person-centered plan or service plan

Be appropriate for a community-based, non-institutional setting

Individuals considered for AFL services typically:

Have a diagnosed intellectual and/or developmental disability

Are eligible for and enrolled in an applicable Medicaid waiver

Benefit from a family-based living arrangement
